Custom File Icons not always working

I have folders on my Desktop that I regularly put things in and out of and also created a test folder structure where the Custom File Icons do not seem to be consistently working. This has been going on for as long as I can remember, and is not a big deal, but just curious if there is something that I can do. I am not using them for Network or Sleeping drives.

Example 001:
You can see that the "_Closed" folder is not consistent between the Tree and the List.

Example 002:
Here you can see the Branch View and that there should be multiple CFI's happening. Sometimes they will work and sometimes they will not.

My configuration dialog:
I tried unchecking the top option for a while to see if that did anything, but everything still is acting the same way. My other Custom File Icons, that I have crossed out, all seem to work just fine, it's just the ones that come with the program.

Hopefully I am just missing something. Thanks for any suggestions.

Current version is: 28.30.0100 (64-bit)
Always using zip file from site, not 'installed'.

Can you give steps to recreate such an inconsistency?

Several times throughout the day I will have multiple folders expanded throughout my Tree, so after I move things in and out of the DR's folder on my Desktop I will use Reset Tree to collapse all the other folders in the Tree. Sometimes it will show the correct CFI for the empty folder(s) and sometimes it will not. I haven't been able to pinpoint any particular differences when trying it. Sometimes I can hit Reset Tree and it will work, sometimes it will not, and as noted before, sometimes the Tree will show the correct CFI but the List will not match. I do have a script that I use to create a new folder under my DR's folder that is on my Desktop, of which I noticed that when I ran it and it created the new folder, the other pre-existing folders that are empty changed from blue (empty) to yellow (has objects (but really doesn't)).

I wish I had more to go on. This is certainly not a big deal, as I can definitely live with the way it is. I just found it odd.

I use CFI on folders all the time, I depend on them, and they seem to work 100% for me. But if I see something odd I will fix it.
