Icon Caching Borked?

I'm getting some weird icon caching issues with 6.30.0044.
I have a program I just compiled. XY shows the icon that was used when I first created it, I then compiled again overwriting the original with an EXE that hasn't changed except for it has a custom icon, and XY doesn't update the icon.
I refresh the cache and it updates to the new icon.
That's all expected behavior and functioning correctly.
What follows, is not..
I then hit F5 in the list and the original icon for the file is back. Refresh the list cache, new icon. Hit Change or Refresh the List View, old icon. Switch tabs and come back, old icon. Switch directories and return, old icon.
The program was also in the catalog and I can't get that entry to update at all unless I restart XY. "Refresh the cache" and "refresh all icons" (they're the same function no?) do not work.
So the catalog cache refreshing for programs just seems busted and the list caching doesn't seem to take.
I tested it with a shortcut as well and that updates properly so it seems to just be an issue with programs.
