Shift-modified hotkeys some times not working

Hello everyone
I've been having this particular issue on and off for a while now, through several versions up to current 17.40.0200. Some times some Shift-modified hotkeys fail to work for no apparent reason.
So in short I used to have a hotkey Shift+D assigned to Miscellaneous / Go To / Open Folder in Other Pane in New Tab it would often fail to do it for some reason, but pressing again once some times twice would eventually open the new tab.
At first I didn't think much of it, but it started happening more and more frequently that it bothered me. I though I just wasn't pressing my keys firmly enough, I've even though about faulty keyboard, but everywhere else works fine. After a while I just though it was some sort of conflict or badly formed key configuration file so rebuilt my ks.dat from scratch.
It now works wonders, I though it solved my problem only to find that it just "moved" it to another place. Now it is Shift-Delete File / File Special / Delete (No Recycle Bin) that fails often. I only noticed after checking my now bloated recycle bin where files I'd swear had deleted.
I've actually had to specifically press it four or five times in a row to actually erase files and not send to Recycle Bin instead, so it is something specific to the Shift key alone, because Delete was working.
I formatted and reinstalled Windows 7 64Bits in the meantime (for other reasons) and it is still there. After further testing it is definitely something related to my profile/user config/combination of settings, because a fresh installation without user settings does not exhibit the same problem.
Any clue what it might be, bad Profile, bad config, some sort of conflict? Corrupted ks.dat or any specific combination of setting that might be causing that?
I know this is probably a very vague and hard to narrow down issue, so would sending any of my settings files help in any way?
