Hello everyone
I've been having this particular issue on and off for a while now, through several versions up to current 17.40.0200. Some times some Shift-modified hotkeys fail to work for no apparent reason.
So in short I used to have a hotkey Shift+D assigned to Miscellaneous / Go To / Open Folder in Other Pane in New Tab it would often fail to do it for some reason, but pressing again once some times twice would eventually open the new tab.
At first I didn't think much of it, but it started happening more and more frequently that it bothered me. I though I just wasn't pressing my keys firmly enough, I've even though about faulty keyboard, but everywhere else works fine. After a while I just though it was some sort of conflict or badly formed key configuration file so rebuilt my ks.dat from scratch.
It now works wonders, I though it solved my problem only to find that it just "moved" it to another place. Now it is Shift-Delete File / File Special / Delete (No Recycle Bin) that fails often. I only noticed after checking my now bloated recycle bin where files I'd swear had deleted.
I've actually had to specifically press it four or five times in a row to actually erase files and not send to Recycle Bin instead, so it is something specific to the Shift key alone, because Delete was working.
I formatted and reinstalled Windows 7 64Bits in the meantime (for other reasons) and it is still there. After further testing it is definitely something related to my profile/user config/combination of settings, because a fresh installation without user settings does not exhibit the same problem.
Any clue what it might be, bad Profile, bad config, some sort of conflict? Corrupted ks.dat or any specific combination of setting that might be causing that?
I know this is probably a very vague and hard to narrow down issue, so would sending any of my settings files help in any way?

Ok, nothing like reporting an error to figure out the cause of the problem.
After a little more testing I seem to be able to recreate the problem reliably. It seems to be related to how "fast" I press the keys, If I press them too quickly it fails often. If I take the time to fully press the Shift key slowly it registers the command flawlessly.
So there must be some sort of "delay" capturing the key presses that makes it fail for fast key presses and work for slower ones. It now only happens for Shif-Delete, Shift+D for duplicating a tab to other pane always works, no matter how fast I press it.

Cannot reproduce (and I'm a fast typer).
The cause is probably that you release the shift key before you press-and-release the other key.
In case of Shift+Delete this sequence will work in any speed:
Shift DOWN
Delete DOWN
Delete UP

Hum, I believe that is not it, there must be something else, because it only happens in XYPlorer, and not in Windows Explorer, and also not with a clean profile.
There must be some other setting or definition causing it, or something got corrupted somewhere in the profile or settings. Maybe a conflict with new setting or features added in the meantime? Who knows...
Anyway, rebuilt all my preferences and settings from scratch, only scripts and catalogs were imported, problem seems to be gone for now. Lets hope it doesn't return
