Maybe this is a setting I changed inadvertently, not sure.
Tools > customize list > show icons is checked. I have been through the tool bar settings and the main configuration file and I don't see the problem.
If I select a file in the list, the icon shows, but only if selected. I have resorted to using labels to visually identify certain folders quickly. That works as well for my purpose, but I was wondering why no icons showing.

Here is a screenshot of what I am seeing, if this helps. The icon only shows in that column if the file or folder is selected.

I've seen this before. It's not an XY issue. Try a windows reboot.

You're correct about this not being an XY issue.
I just changed my theme to a high contrast one, and then back again to the Aero theme I had been using. I did not do this to try and resolve this problem; totally unrelated. I did however want to change the blue background color of a selected file in the list, as shown in the screenshot of my previous post, so while I was in appearances, I did so.
A bit later I opened XY to organize some files, and lo and behold, I now see the icons. Can't figure why that fixed it, but maybe this will help someone else out if they are having the same problem.
