I don't think you can assign an icon to a specific file, although I may be wrong.
You can assign an icon to all mp4 files by following the instructions you used when you asked the same question about jpg files.

Or being more precise: Instead of using just a file extension in your Custom File Icon definitions, you can enter more specific details including full file specifications, too.

Not sure if I understand the question, but in the examples given, just replace the extension by the full mp4 file name... should do the trick

Why not trying ?
(Of course this works.)
i.e
Full_filename_of_program>fulfilename_of_ico_file
Note that once you've entered the program name (typing or use Browse button), simply double-click on the line and XY will open file requester to chose an icon.
