I continue to have intermittent problems with XY refreshing the List items.
The image below shows the same icon folder as viewed with with XY on the left and File Explorer on the right.
I had made a copy of the included blue XY icon as XY Green.ico, changed the color accordingly, and saved.
The updated color was seen immediately in File Explorer but not in XY even after toggling all the XY View\Refresh options and restarting XY.
(Note this is not relative to changing the assigned XY icon, only to icons appearing in the right pane List)

2) Your Windows Version (e.g., Win 11)
3) Your Screen Scaling Percentage (e.g., 125%).

Yes, this is a known problem with Windows. Windows keeps a "system image list" cache (that's where all system icons are stored) per process (even across kills and restarts of the same process), and it can get out of sync with the facts. I don't know of any way to force this cache to be updated (other than a cold Windows restart). Only ICO files are affected by this.

Maybe File Explorer doesn't use the cache and always does a direct read?

Maybe. Every process has its own list (cache). I have not tested this now, but years ago it was possible to confuse Explorer by doing some deletions and renames.
