In those cases, it is probably drawing it from the windows cache not from the exif directly, hence why moving the folder resolves it.

I see... lots of caching going on.
To add one point: not updating / rewriting the embedded thumb may be per a philosophy of keeping all original info, especially for camera shots. To update this one part of the EXIF info would actually be an exception since AFAIK nothing else would or should be changed when an image is rotated or cropped etc.
So - for XY to guide the user a little more in the choice of thumbs Quality / method would be appropriate. And as I mentioned this could be the underlying reason for some "thumbs don't update" reports.
Since my graphics app (FastStone Image Viewer) doesn't even have an option to update the embedded thumb, I 'll stick with XY's Crisp or Smooth quality levels for now.
[But I DO want the embedded thumb to be updated, since a crop is usually to remove unwanted / insecure / private material. So I might have to find a new image viewer / editor now that I understand more of what's happening].
