1) Faulty 3rd party shell extensions.
2) PDF preview using Adobe preview handlers.
Number 1 can be worked around by ticking Configuration | General | Menus, Mouse, Usability | Context Menus | Hide shell extensions from shell context menu.

Just to post an update to anyone that might face the same issue. The problem of random crashing seems to have resolved by uninstalling Adobe Reader DC and installing Adobe Reader XI. Simply repairing the install of Adobe Reader DC wouldn't fix the issue.

For information, regarding what I posted before here (dllhost hanging when video thumbnails are generated), it was indeed an issue from the Icaros side.
It has been fixed in version 3.2.1.
