Tested and working. But one caveat:
Since the option is in the 'General' section, we are losing the flexibility we had to have two different behaviors for Left-MDBU and Right-MDBU.
Previously, I was using Left-MDBU to quick preview the image at full size (with full panning), and Right-MDBU to preview at Fit-to-Screen.
Since 'On Right Mouse Down' already has a set of distinct options, can the option be moved to apply only for Right-MDBU? That way we can retain the distinct behaviors for the two mouse buttons.
It could be a sub-option under: On right mouse down -> Fit Image to Screen -> Fit Width Only

@klownboy: Cannot reproduce. Could post a screenshot of your MDBU settings?

Ideally, the behavior would be:
1) When you Left-MDBU: Image blows up to full size, with full 4 direction panning
2) When you Right-MDBU: Image blows to to 'Fit Width' with only vertical panning
The settings:

Hmm, well that makes sense. Couldn't you come up with this earlier...?
Wait, no! This setting is only for
Mouse Down on Thumbnails and Icons, not for MDBU on other previews.
What we could do is change "Fit width only" to "On right mouse down fit width only".
No, nonsense. There is no right-mouse MDBU on other previews.
BTW, my "Cannot reproduce." was meant for
klownboy. My mistake. I made it clearer now in the post.

Don, try these settings on a thumbnail, On Right mouse down and Stay up checked and Fit image to screen unchecked. On right click, the blow up image is Fit to the screen and fixed at the top of the image with no movement possible. See attached.

Also with On Right mouse down, Stay up, and Fit image to screen all checked, no movement is possible on right mouse down. Was movement or scrolling intended with Stay up checked?

I think klownboy's situation is also the same issue I pointed out. The 'Shrink to Fit' option in the 'General' section overrides the 'Mouse Down on Thumbnails and Icons' section, even when 'On right mouse down' -> 'Fit Image to Screen' is unchecked. So both left and right MDBUs are 'fitting to width'.
I think the solution you mentioned you've come up with (to have the distinct behaviors) will also solve what klownboy is describing.

I had to run off to get a tooth drilled, but what jaywalker32 stated is correct. "Shrink to Fit Screen" in the General is affecting the On right mouse down setting for Shrink to Fit. When General setting Shrink to Fit is checked and On right mouse down setting for Fit Image to screen is unchecked, you'll get A MDBU shrunk to fit on right click which is what I was originally pointing out.
Also noted with:
General settings:
Shrink to fit and Fit width only both unchecked.
On Right mouse down: Stay up checked and Fit image to screen unchecked.
I can right click (no hold) and the image comes up and stays up and does not move if I attempt further mouse actions. if I right click keeping the mouse held down, the image moves. Maybe this is expected behavior (or not)?

I'm still seeing the same 2 issues in my last post (actually the first one would be considered an issue, the second is more a question on Stay up with a single right click (not held) vs right click held down and movement). Note: I don't use these settings so it's not a huge deal for me. I just wanted to let you know.

I didn't change any settings in MDBU, and now when shrink to fit is enabled, no movement in any direction is possible anymore for images/videos (pdf/text files still move), is that correct? I thought no vertical movement only applied to the new fit width option?.
