Find file is showing 2 entries for the same file

It is not showing two entries for all files, just some. Do not know why these files show two entries

And you really think that this statement alone helps on debugging this issue?

I am doing a tag search, so I took a look at the "Tagged Items" file. There are two entries in the tag file for the same file. Not sure how that would come about, but I think that is why the tag search displays two lines for the same file.
