Does work fine in a VM...

Show a screenshot of Menu - Tools - Configuration - Preview - Previewed Formats - [x] Video Files (scrolled to the top)

and additionally try a fresh instance

Working now and looks like I needed the k-lite codecs. Was clue-less about this since XY previewed all the other formats.
Why does MP4 need separate codecs?

.mp4 is just a container for video / audio data. It can contain a lot of stuff (h264, h263, vc-1, etc.)
So to see if this is / was really a problem of the inbuilt mp4 codec of the Windows OS, you'd need to
upload the file / a non-reencoded part of it...

I tried to upload the MP4 file but it was blocked as an invalid file extension. Can you explain "non-reencoded part of it"?
Thanks

The forum itself isn't the right place to upload videos :)

Use any filehoster (mega, dropbox, googledrive, whatever)

You need a tool that doesn't change any relevant codec params when cutting it into pieces
I for myself use ffmpeg (a command line tool) for such things but "mp4 split no reencoding"
as a google search should reveal a lot of tools.

Most of the MP4 files I have were downloaded from YouTube, and none have been modified by me.

Thanks for the link.

No, it's not a codec issue. XY does preview this file as well without any installed external codec(s).

So there is only one logical conclusion: A 3rd-party software product on your computer (my guess:
your player that is registered for .mp4 files) left the original mp4 codec in a non 100% working state
and the k-lite codec package installation fixed that issue

I have MP4 associated with VLC and the others with Cyberlink; so the MP4 codec alone could have been damaged and the other formats left intact?

Judging from your former screenshot(s), Cyberlink was associated with .mp4, not VLC.
Cyberlink has been (haven't used it in years because of that) a monster regarding the modification
of registry entries and sure, one codec can be non functional while others are working fine

Right, forgot I changed it in my debugging; I just set it back to Cyberlink and it still works so perhaps the k-lite codecs are keeping it safe.
All is well so thanks for the help!
