I don't open a new thread for it, but the very same happens in the CFA editor: I have just browsed for a file using double-click and when I wanted to test it I noticed it disappeared from the CFA menu. Double click unticked the entry.

Indeed, I can confirm that. Had to play with pin/unpin first really.

I would appreciate if clicking on a catalog item would refresh the icon. It could reflect if a file has been deleted (icon would change to question mark) in the meantime or restored. (It could keep the icon of the used items fresh, without the necessity of the manual full refresh of catalog icons.)

Do not overthink it. Select txt files by for example the Selection Filter *.txt, then simply press <Enter>.
If you want more customization, check in the help file what Custom File Associations (CFA) is and also the "Open with..." menu item.

Actually, the breadcrumb bar has the very same functionality already.
